Nursing Home Admission & Pricing
Admission into the nursing home is easily started by calling and speaking with the Social Worker, Warden Weaver, or the Executive Director, Mary Carroll. You are encouraged to tour the facility and to sit down with our staff to discuss your situation.
Medical records will be reviewed for potential needs and to determine appropriateness for admission.
Prior to, or at the time of admission, copies of any advance directives and insurance information will be required.
For admissions into skilled nursing following a qualifying hospital stay, the hospital discharge planner/case manager can handle the referral and send the information needed to Golden Age Home.
