Golden Age Home partners with Hallmark Rehabilitation for a team consisting of physical therapists (PTs), occupational therapists (OTs), and speech language pathologists (SLPs) who work together to provide focused, individualized treatment and rehabilitation for our patients.

  • Physical therapy addresses posture, balance, strength, walking, fall prevention, positioning, and wound care.
  • Occupational therapy focuses on daily activities such as bathing, eating, dressing and toileting as well as instrumental activities of daily living such as home management and community reintegration.
  • Speech therapy addresses swallowing problems, memory disorders, safety awareness and communication challenges.

All treatments are provided to regain the highest levels of function. The patient and their family members are actively involved in all aspects of the rehabilitation process, from goal setting to discharge planning.
